Dear Parents,

If you have received a Weapons Form or a Concussion Form you must fill it out because I have not received one back from you or it has been lost in transit.  The concussion form is simply explaining what a concussion is and what the symptoms are and letting you know you will be notified if your child shows signs or symptoms of one after a head injury. If it is not signed and returned your child cannot participate in recess, gym, field trips and any other physical activities that may take the child outside or in the gym to perform.

Field Trip is Wednesday, please dress your child according to the weather as we will be outdoors.  Make sure your student is on time for school that day.  These parents have volunteered:

Chaperons please arrive to our classroom at 9:00 so that I can give you your list of students  and any other information that may be needed about the trip. Thank you in advance for your help.  If I have missed anyone who has volunteered please let me know.
