As we start a new week & a new year tomorrow, I would like to share a message from our school’s principal, Mrs. Tammy Fournier regarding staff, students & families’ health, and safety.

“Please help us keep our students healthy and in school by doing the following:

• If your child or anyone in your household has been exposed over the Holiday Break to Covid, please keep your child home from school and contact the school office for further guidance.
• If your child or anyone in your household has any symptoms of illness or has tested positive for covid, please keep your child home from school and contact the school office for further guidance.
• If your child or anyone in your household is unable to pass the Daily District Screener, please keep your child home from school and contact the school office for further guidance.
• If your child or anyone in your household has a covid test result that is pending-either rapid or PCR, please keep your child home from school and contact the school office for further guidance.

We need every individual to follow this advisory message in order for us to help mitigate the spread of Covid-19 within our school community.

COVID-19 screening tool 

Staff and visitors, if you answer “yes” to any of these questions, you must stay home from school today. Parents and guardians, if your child would answer yes to any of these questions, they must be kept home from school. 

Question 1: Do you have any new, unusual, or worsening symptoms listed in Boxes A and B  below? 

Box A 

∙ Fever? (100.4°F or higher) 

∙ Cough? 

∙ Shortness of breath? 

∙ Difficulty breathing? 

∙ Loss of smell or taste? 

Do you have at least one of these symptoms? □ Yes □ No 

Box B 

∙ Vomiting or diarrhea? 

∙ Headache? 

∙ Sore throat? 

∙ Runny nose and/or congestion? 

∙ Body aches and/or tiredness? 

Do you have at least two of these symptoms? □ Yes □ No 

If you answered “yes” in either box above, then stay home and consult your primary care doctor. If a  doctor determines the symptoms are due to another diagnosis, or if COVID-19 is ruled out, you may return to school after at least 24 hours fever-free without the use of fever-reducing medicine and without vomiting. 

Question 2: In the last 10 days have you been identified as close contact with anyone who has had a positive COVID-19 diagnostic test? You may answer “No” if: 

∙ It has been more than two weeks since you received the second dose of the Pfizer or Moderna vaccine or  one dose of the Johnson and Johnson vaccine 

∙ Within the past five months, you had a positive PCR test for COVID unless the test was within the last 10  days. 

□ Yes □ No 

If you answer “yes”, then stay home to quarantine for at least 10 days from the last exposure to close contact. In some situations, you may be able to return sooner with regular COVID testing. See our current quarantine guidelines. If symptoms appear, you must stay home for at least 10 days after your symptoms start and until any cough, fever or other symptoms have improved. (See symptoms section above.)  

Question 3: Have you returned from international travel in the last 10 days? You may answer “” No” if you had a negative COVID test (PCR or rapid) after returning to the United States. 

□ Yes □ No 

If you answer “yes”, then stay home until you have a COVID test and the results are negative.

If you test positive for COVID, please report that information to your school office as soon as possible to assist with contact tracing. For questions, contact the Wayne County Health Department at (734) 727-7078,

الموظفون والزوار الكرام ، إذا أجبتم بـ “نعم” على أي من األسئلة التالية، فيجب عليكم البقاء في المنزل و  عدم الحضور إلى المدرسة اليوم. اآلباء واألوصياء ، إذا أجاب طفلكم بنعم على أي من هذه األسئلة ، فيجب  إبقائه في المنزل بعي ًدا عن المدرسة. 

السؤال األول: هل تعاني من أعراض جديدة أو غير عادية أو تدهور في األعراض المدرجة في الجدولين أ و ب أدناه؟  

الجدول ب 

∙ إقياء أو إسهال؟ 

∙ صداع ؟ 

∙ التهاب في الحلق؟ 

∙ سيالن في األنف أو احتقان؟ 

∙ آالم في الجسد و \ أو إرهاق؟ 

∙ هل تعاني من اثنتين على األقل من هذه األعراض ؟ □ نعم □ كال  

الجدول أ  

∙ ارتفاع في درجة الحرارة ؟ ) درجة الحرارة فهرنهايت ٤ ، ١۰۰ أو ما فوق( 

∙ سعال؟ 

∙ ضيق في النفس؟ 

∙ صعوبة في التنفس؟ 

∙ فقدان حاسة الشم أو التذوق ؟

اذا أجبت ” بنعم ” على أحد األسئلة في أي من الجدولين ، ابقى طفلك في المنزل واستشر طبيب الطفل أو مقدم الرعاية الخاص به. اذا قرر الطبيب  أن أعراضه نتيجة تشخيص لمرض أخر، أو استبعد اإلصابة كوفيد -١١يسمح عندها لطفلك بدخول المدرسة بعد مرور ۲٤ ساعة على تعافيه من  األعراض دون استخدام األدوية الخافضة للحرارة أو إقياء.

السؤال الثاني: خالل العشرة أيام السابقة، هل كان الطفل على اتصال قريب من أي شخص تم تأكيد إصابته بكوفيد -٩١عبر  نتيجة فحص إيجابية ؟ يمكنك اإلجابة بـ “كال” في حال: 

∙ مضى أكثر من أسبوعين منذ أن تلقيت الجرعة الثانية من لقاح فايزر أو موديرنا أو جرعة واحدة من لقاح جونسون وجونسون. ∙ خالل األشهر الخمسة الماضية ، قمت باختبار PCR و كانت نتيجته إيجابية تؤكد اإلصابة بكوفيد ، ما لم يكن االختبار خالل األيام العشرة الماضية. □ نعم □ كال  

اذا كانت اإلجابة بنعم ، يجب أن يبقى طفلك في البيت للحجر الصحي مدة عشرة أيام على األقل بدءاً من تاريخ اختالطه بالشخص المصاب قد تتمكن  في بعض الحاالت من العودة سريعًا من خالل القيام باختبار كوفيد- المنتظم – الرجاء االطالع على دليل الحجر الصحي الخاصة بالقطاع التعليمي. في  حال ظهور األعراض يجب عليك البقاء في المنزل لمدة عشرة أيام على األقل بعد بدء األعراض وحتى تتعافى من أي سعال أو حمى أو أعراض أخرى. )انظر قسم األعراض أعاله( 

السؤال الثالث: هل عدت من رحلة دولية خالل العشرة أيام األخيرة؟ يمكنك اإلجابة بـ “كال“ إذا قمت باختبار كوفيد سريع أو  عادي و جاءت نتيجته سلبية بعد عودتك إلى الواليات المتحدة األميركية.  

□ نعم □ كال  

إذا كانت اإلجابة ” نعم“ يجب عليك البقاء في المنزل حتى يتم إجراء اختبار كوفيد وتأتي نتيجته سلبية. 

إذا جاءت نتيجة اختبار الكوفيد إيجابية ، فيرجى إبالغ مكتب مدرستك بهذه المعلومات في أقرب وقت ممكن للمساعدة في تتبع جهات االتصال. للسؤال و  االستفسار الرجاء االتصال بقسم الصحة العامة لمقاطعة وين على الرقم 7078-727)734(

Empleados y visitantes, si contesta “si” a cualquiera de estas preguntas se debe  quedar en casa hoy y no ir a la escuela. Padres o guardianes, si su hijo(a) contesta  sí a cualquiera de estas preguntas, deben quedarse en casa y no ir a la escuela.  

Pregunta 1: Tiene algún síntoma nuevo, inusual, o que haya empeorado que este  mencionado en la lista de la Caja A y Caja B? mencionado en la lista de la Caja A y Caja B?  

Caja A 

Fiebre? (100.4°F o mas alta) 

Tos? 

¿Corto la respiración? 

¿Dificultad para respirar? 

Pérdida del olfato o del gusto? 

¿Tiene usted al menos uno de estos síntomas? □Si □No 

Caja B 

Vomitando o diarrea? 

Dolor de cabeza? 

Dolor de garganta? 

Secrecion nasal y/o congestion? Dolores de cuerpo y/o cansancio? 

¿Tiene usted al menos dos de estos sínto mas? 

□Si □No

Si contesto “si” a cualquiera de estas preguntas en cualquiera de las cajas de arriba, entonces quédese en su  casa y consulte a su doctor de cuidados primarios. Si su doctor determina que sus síntomas son debido a otro  diagnóstico, o si Covid-19 está descartado, entonces puede regresar a la escuela después de al menos 24 horas que  esté libre de fiebre, sin el uso de medicamentos para reducir la fiebre y sin estar vomitando. 

Pregunta 2: En los últimos 10 días ha sido usted identificado como alguien que haya tenido  contacto cercano con alguien quien ha tenido una prueba de diagnóstico que haya resultado  positivo con el Covid-19? Puede contestar “No” si acaso  

∙ Han pasado más de dos semanas desde que usted recibió la segunda dosis de vacunas de Pfizer o  Moderna o una dosis de la vacuna de Johnson y Johnson.  

∙ Dentro de 5 meses usted tuvo una prueba de PCR o COVID positivo, al menos que la prueba haya sido  en los 10 últimos días.  

□Si □No ∙ 

Si usted contestó “si”, entonces manténgase en su casa bajo cuarentena por al menos 10 días desde la última vez  expuesto a contacto cercano.En algunas situaciones puede regresar antes con la prueba regular del Covid. Vea  nuestras pautas actuales de cuarentena. Si aparecen síntomas, se debe quedar en casa por un mínimo de 10 días  después que sus síntomas comenzaron y hasta que cualquier tos, fiebre, u otros síntomas hayan mejorado. Pregunta 3: Ha usted regresado de viajes internacionales en los últimos 10 días? Usted  puede contestar “”No” si usted ha tenido una prueba de Covid-19 negativa (PCR o rápida) después  de haber regresado a los Estados Unidos.

Si contesta “si”, entonces quédese en su casa hasta que haya tenido una prueba COVID y los resultados sean  negativos.
