We will be going to the zoo on Wednesday, May 27^th.  If you would like to be a parent chaperon you must agree to the following:

1. Fill out the Volunteers and Chaperons Background check form (attached) and return to OFFICE (they must see your driver’s license or photo ID) by May 1^st

 

2. Be willing and able to be responsible for a group of 2-4 students (depending on number of parents attending)

 

 

3. Be willing and able to drive your own car or carpool with other parents (you will need to pay $6.00/ car to park)

 

4. The cost for parents is $5.00 so please send in $10.00 for you and your child by May 15^th

 

 

5. You may not bring other children with you.

يذهب صف الروضة في رحلة الى حديقة الحيوانات وذلك نهار الاربعاء 27 ايار\مايو,  يجب ان توافقوا على ما يلي اذا رغبتم بالذهاب مع التلاميذ للمساعدة :

 

 

1. املاء استمارة في الادارة للتحقق من شخصيتك . وابراز دفتر القيادة بمدة اقصاها 1 ايار\مايو.

 

2. تحمل مسؤولية 2-4 اولاد في مجموعتك.

 

 

3. القيادة بسيارتك واخذ بعض الافراد معك دفع 6 دولارات لتوقيف السيارة)

 

4. تكلفة الرحلة 5 دولارات للشخص لذلك نرجو منكم ارسال 10 دولارات لك ولولدك.

 

 

5. الرجاء عدم احضار الاطفال معكم.
