This school year, we will be planning a couple of field trips to various places. We ask that parents drive and chaperone their own child to and from these field trips. In order to do this, our school must have a “Volunteers and Chaperones Criminal Conviction History Form” completed, signed and in the office at least 10 days before a trip. Let’s take care of this form now. I will be sending the form home today in your child’s Red Communication Folder. Please fill it out, sign it and return it in the Red Communication Folder. We also will need a copy of your driver’s license. This form will only need to be completed once this school year. This is for the safety of all our students. Thank you.
