Your first name, last name and Teacher’s name must be printed on your answer sheet.
Print NEATLY, please, so I can read your writing.
Fold your answer sheet and place it in the box right outside the locker room doors!
ONLY ONE ENTRY PER STUDENT!
You must turn in your answer sheet in the box by: Tuesday, April 29th – 3:40 pm!!!!! No answer sheets will be allowed after that time!
