Updated Locker Room Clean Out Form

Hi everyone. Since most of you do not remember or know your locker number, we have created a form that has a map of both the Girls and Boys locker room. I EMAILED EVERYONE an attachment of the locker room form which you HAVE to fill out. Here’s what you need to do:

1.Print out the form and fill it out completely, including circling the area of the locker room your locker is on the map.

2.  Scan or take a picture on your phone and send it to Mr. DelGiudice at delgiuj@dearbornschools.org

3.  Must be sent by noon on Friday, 5/29.

4.  Contents will be bagged and available for pickup from 10-12 on Monday, June 1st.

5.  All remaining items and locks will be discarded.

6.  If you have stuff in another locker besides the two above, please send an email to either your coach if you are athlete, or Mr. DelGiudice providing information on the form to them.
