You can follow the above link to access the iChat form! We cannot have you volunteer in the classroom until this form is submitted and processed through the district. If you have already submitted an iChat for another child within Dearborn Public Schools, we do not have to have you repeat these steps. Here are the steps to take to fill out and submit:

1. Print out the form and fill it out OR I have paper copies available
2. Print out a copy of your Driver’s License/State Issued ID front and back OR you can email me a picture if you do not have access to a printer
3. Bring the completed form and the copies of your Driver’s License/State ID and we will fill out the rest together
4. I will take the completed form and submit it through the district and let you know when it is fully processed
