Ages and Stages Developmental Screening Parent Letter/Instructions

Dearborn Public Schools GSRP is taking part in a county wide project called Help Me Grow, through Wayne County.

This project focuses on making sure your child is on track and identifying areas where additional support may be needed.  This is an online tool that only takes about 10 minutes to complete.

Information gathered from your answers will be used by your child’s teacher to support their growth and learning.

Your child’s specific information will not be made available to the general public.  Your personal information (email, address) will not be used for any other purpose.

In order to complete the screening, go to

  1. First enter in your child’s birthdate (month, day, and year).
  2. Next, enter in the number of months your child was premature.  If none, enter “0” (there must be a number in this area to continue).
  3. On the next screen, in step 1, you will pull up the screening tool on a PDF document.  You can (a) review the questions on the screen and decide whether you are able to do the screening without further work with your child OR (b) print the questions and work with your child on those answers you are unsure of.
  4. Once you are sure of the answers, return to that screen and click on step 3.
  5. Next you will need to answer the demographic information for your child.
  6. Finally you will complete the screening questions on the screen.  When done, submit the screen for review.
  7.  For your records and the teachers please print a copy of the confirmation page.

NOTE: you are not able to save and come back to your responses-you will need to complete the full screening at one setting.
