COVID-19 Screening Tool  for Families <\/p>\n\n\n\n

Parents and guardians, use this checklist every day before sending  your children to school.  <\/p>\n\n\n\n

If you answer \u201cYES\u201d to one or more questions, you <\/em><\/strong>must <\/em><\/strong>keep your child  home from school today. <\/em><\/strong><\/p>\n\n\n\n

Does your child have any new, unusual, or worsening symptoms  as listed in Box One and Box Two below? <\/p>\n\n\n\n

Box 2 <\/strong><\/p>\n\n\n\n

\u2022 <\/strong>Vomiting or diarrhea? <\/strong><\/p>\n\n\n\n

\u2022 <\/strong>Headache?  <\/strong><\/p>\n\n\n\n

\u2022 <\/strong>Sore throat?  <\/strong><\/p>\n\n\n\n

\u2022 <\/strong>Runny nose and\/or congestion? <\/strong><\/p>\n\n\n\n

\u2022 <\/strong>Body aches and\/or tiredness? <\/strong><\/p>\n\n\n\n

Does your child have at least <\/strong>1 <\/strong>of these symptoms? <\/strong> <\/strong>YES NO <\/p>\n\n\n\n

Does your child have at least <\/strong>2 <\/strong>of these symptoms? <\/strong> <\/strong>YES NO <\/p>\n\n\n\n

If the answer to any of the questions above is \u201cyes\u201d, keep your child home <\/strong>and consult your primary care physician.  If a doctor determines that the symptoms are due to another diagnosis, or COVID-19 is ruled out, your child may return to  school after being fever-free for 24 hours without <\/em><\/strong>the use of fever-reducing medications. <\/p>\n\n\n\n

In the last 10 days, has your child been identified as a close contact of anyone who has had   a positive COVID-19 diagnostic test? <\/strong><\/p>\n\n\n\n

If the answer to this questions is \u201cyes\u201d, your child must stay home <\/strong>to quarantine for at least 10 days from the last  exposure to the close contact unless symptoms appear (see section above regarding symptoms in Box 1 and Box 2. <\/p>\n\n\n\n
