Title of Book (1 points) 2. Author (1 points) 3. Lexile (1\/2 point) 4. Complete the \u201cSummary Form\u201d and use it to write a detailed summary on the form provided. (25 points) 5. Provide a character analysis of the protagonist(s) and antagonist(s) of your book. Be sure to include the characters names and their traits. A visual to accompany this information would be great. (10 points) 6. Find a way to creatively write a THEME for your book and explain how this THEME was revealed in the story. (5 points) 7. Rate your book 1-4 stars (4 being the best) and explain the rating. What was awesome about the book and\/or what would you change? (5 points) 8. Write about your favorite scene from the book! Explain what happened and tell why it was your favorite scene.
